Data Collection Tools
IFMC IS leads the way in creating specifications for data collection and reporting of clinical measures. Depending upon your program objectives, IFMC's team of health informatics and systems expertise define the information and business requirements necessary to capture the data.
Our process consists of:
- Identifying the program measurement and reporting requirements
- Defining the data attributes for measurement calculations or reporting
- Building data collection tools that minimizes input costs while maximizing data capture
- Incorporating data vocabulary standards where applicable
- Validating and conditioning data used for reporting
Hospitals - CART : CMS Abstraction and Reporting Tool
CART, the CMS Abstraction & Reporting Tool, is a powerful application
for the collection and analysis of quality improvement data. Through
data collection, retrospective analyses and real-time reporting, CART
enables hospitals to comprehensively evaluate and manage quality
improvement efforts.
CART can be used for collecting data to measure CMS quality indicators in the clinical areas of Acute Myocardial Infarction (AMI), Heart Failure (HF), Pneumonia (PN), and Surgical Infection Prevention (SIP).
For more information on the CART tool, refer to www.QualityNet.Org.
Physician Offices - iRegistry : Patient Data Collection and Quality Measurement Tool
iRegistryT provides health center offices the core functionality needed to document and
capture patient information while incorporating quality management work processes at
the time care is administered to patients.
iRegistry is an advanced COTS registry system that supports the federal government's Consolidated Health Initiative (CHI) aimed at standards adoption through the implementation of system interoperability and data vocabulary standards.
iRegistry is a Microsoft Windows© based system which resides locally or within a local area network. Operating systems include Microsoft Windows© 98, NT, 2000, and XP. Data can be transmitted to a centralized data warehouse which is used for pay for performance programs or performance analysis.

Any Payor or Healthcare Entity- iHealthForms MD : Survey & Questionnaire Tool
Driven by rapidly changing customer demands and growing response to capture data used in clinical settings,
IFMC developed iHealthForms MDT.
iHealthForms MDT is essentially software that automatically generates XML business rules authored by clinical users.
The resulting visual representation is a survey tool consisting of a series of question and answer combinations that guide users to further questions based on their responses.
iHealthForms MDT can be used in a variety of settings ranging from healthcare quality reporting, longitudinal studies, and disease case management for disease surveillance monitoring. Its flexibility can be extended to any survey or data collection program.
